Being a Sponsor for Confirmation (like Baptism) does not only mean that you have made your First Holy Communion, First Penance (Confession) and have been Confirmed, but it also means (as seen below according to Canon Law) that this person should be living a life that is "in harmony with the faith and the role to be undertaken." This specifically and realistically means going to Mass reguarly.
In order to have this form signed by Saint Joseph Church, you will need to be an officially registered member of the parish (which means completing the new Census Form which was instituted this past year), and attend Mass weekly (not weakly) as well as being a good example of Christian living for the young adult to be confirmed.
Often times, siblings or friends that are away attending college are asked to be Sponsors for a Confirmation student. If this is the case, then this intended Sponsor must go to the Chaplain of the College or University where they attend and have the campus priest sign this document. Being a "former memeber" of a parish or coming to church "when you come home" is not sufficient for being a Sponsor.
